Foreign residents are required to obtain a re-entry permit if leaving Japan and planning to return with the same visa.
There are 2 types of re-entry permit; SINGLE and MULTIPLE re-entry.

Application fee (as of December 2008)
Single re-entry permit : 3,000 yen / person
Multiple re-entry permit : 6,000 yen / person
A multiple re-entry permit is valid until the date of expiration of your visa, or for a maximum of 3 years.
16 years of age and older applicants must appear in person to the Immigration Office near you.
What to bring:
1. Application Form
2. Certification for payment of fee*
2. Passport with valid visa attached
3. Alien Registration Card or Certificate of Alien Registration
*you need to purchase 3,000 yen / 6,000 yen revenue stamp (Shu-nyu-inshi) from post office or convenience store and attatch to the certificate.
It can be purchased at ‘ampm’ convenience store on the ground floor of Tokyo Immigration Office.
